Privacy Notice
In line with the Data Protection Act 2018 (DPA 2018) and UK General Data Protection Regulation (UK GDPR), and as a professional registered with the Information Commissioner’s Office (ICO), I am obliged to share the following information regarding how your data is protected:
Information held
As a therapist in private practice I collect the following information: your name, address, phone number, email address, date of birth, next of kin and GP details. The data collected is used to enable effective communication during the therapeutic process. I will use your preferred method of contact to get in touch with you regarding appointments and to reply to you if you make contact with me.
Secure storage and Information sharing
The information you give me when we contract to work together is stored securely in electronic format. Anonymised phone numbers and email addresses are stored on my telephone and/or computer.
Your contact details are held securely for up to 1 year after the therapeutic process has ended. Your name and session notes will be held securely and confidentially for 7 years after the therapeutic process has ended, at which point they will be deleted or destroyed accordingly.
It may become necessary to share your data with a third party if I feel you, or someone else you know, is at risk of significant harm. Unless the risk is imminent, this will be discussed with you before appropriate disclosure. I do have a legal obligation to break confidentiality in compliance with a court order, concerns over child protection and information or knowledge regarding serious crimes or acts of terrorism.
Right to Access/ Erasure
You have the right to ask for a copy of the information I hold about you, also the right to ask me to amend or change any incorrect information about you. You have the right to ask me to erase information I hold about you. In all cases and when considering such requests, these rights are obligatory unless it is information that I have a legal obligation to retain.
